    Abstract [en]

    Internet-based mental health services increase rapidly. However, national surveys are incomplete and the consequences for such services are poorly discussed. This study describes characteristics of 60 Internet-based mental health services in Norway and Sweden and discusses their social consequences. More than half of the services were offered by voluntary organisations and targeted towards young people. Professionals answered service users questions in 60% of the services. Eight major themes were identified. These characteristics may indicate a shift in the delivery of mental health services in both countries, and imply changes in the understanding of mental health.

    Abstract [en]

    Traditional research and literature on project management and organisation theory tend to view project organisations as non-political bodies and purely action-oriented endeavours. In contrast, this paper presents an alternative analysis drawing on the idea of projects as political and emergent processes. Based on in-depth, case-study findings of a complex development and implementation project, we suggest an analytical framework that focuses on the interrelatedness of action and political processes and which explains how project management deals with the two processes simultaneously. We identify and analyse three separate but nested organisational logics applied by the project management team to cope with the dual challenges of politics and action. The general idea is to illustrate the notion of projects as emergent processes involving both politics and action. The three logics are: 1) balancing openness and closure; 2) reformulating tasks to seek solutions; 3) relating to improve action capacity. Our findings add to the literature on the role and practice of project management in complex projects that entail both stakeholder and technological challenges.

    Objective: The aims of the present study was to investigate communication impairments in a Norwegian sample of children with ADHD and children with Asperger syndrome (AS) and to explore whether children with ADHD can be differentiated from children with AS in terms of their language profiles on the Norwegian adaptation of the Children’s Communication Checklist Second Edition (CCC-2). Method: The CCC-2 was completed by the parents, and altogether, 77 children aged between 6 and 15 years participated in the study. Results: Communication impairments were as common in a group of children with ADHD as in a group of children with AS. Although a similar pattern appeared on most CCC-2 scales, children with ADHD and children with AS could be distinguished from each other in terms of their language profiles on the subscales assessing stereotyped language and nonverbal communication. Conclusion: Language abilities should be taken into account when standard assessments of ADHD and AS are performed and before therapies are initiated

    The usability of a Norwegian adaptation of the Childrens Communication Checklist Second Edition (CCC-2) in differentiating between language impaired and non-language impaired 6- to 12-year-olds. Scandinavian Journal of Psychology.

    The aim of the present study was to evaluate if the Norwegian adaptation of the Childrens Communication Checklist-2 (CCC-2) differentiates between a language impaired and a non-language impaired population and to make a first evaluation of the psychometric qualities of the questionnaire on a Norwegian sample. A total of 153 children aged 6-12 years participated in the study (45 language impaired and 108 non-language impaired). The Norwegian adaptation of the CCC-2 distinguished language impaired from non-language impaired children and thus seems to provide a useful screening tool for communication impairments in Norwegian children. The reliability of the CCC-2 appeared to be reasonable with internal consistency values ranging from 0.73 to 0.89.

    Objective: This study aimed to explore whether children with specific language impairment (SLI) and children with ADHDcan be differentiated from each other in terms of their language profiles, and also to investigate whether these two clinicalgroups differ regarding mental health problems. Method: A total of 59 children in the age range 6 to 12 years participatedin the study. The parents completed the Children’s Communication Checklist–Second Edition and the Strengths andDifficulties Questionnaire. Results: Communication impairments were as prominent in the ADHD group as in the SLIgroup; however, the groups were separable from each other in terms of their language profiles. Furthermore, the ADHDgroup experienced significantly more mental health problems compared with the SLI group. Conclusion: Language shouldbe assessed in children with ADHD and instruments sensitive to ADHD should be included when assessing children withSLI.     This study investigated language function associated with behavior problems, focusing on pragmatics. Scores on the Children’s Communication Checklist Second Edition (CCC-2) in a group of 40 adolescents (12–15 years) identified with externalizing behavior problems (BP) in childhood was compared to the CCC-2 scores in a typically developing comparison group (n=37). Behavioral, emotional and language problems were assessed by the Strengths and Difficulties Questionnaire (SDQ) and 4 language items, when the children in the BP group were 7–9 years (T1). They were then assessed with the SDQ and the CCC-2 when they were 12–15 years (T2). The BP group obtained poorer scores on 9/10 subscales on the CCC-2, and 70% showed language impairments in the clinical range. Language, emotional and peer problems at T1 were strongly correlated with pragmatic language impairments in adolescence. The findings indicate that assessment of language, especially pragmatics, is vital for follow-up and treatment of behavioral problems in children and adolescents.

    Abstract [en]

    A new treatment form has emerged that merges cognitive behaviour therapy with the Internet. By delivering treatment components, mainly in the form of texts presented via web pages, and provide ongoing support using e-mail promising outcomes can be achieved. The literature on this novel form of treatment has grown rapidly over recent years with several controlled trials in the field of anxiety disorders, mood disorders and behavioural medicine. For some of the conditions for which Internet-delivered CBT has been tested, independent replications have shown large effect sizes, for example in the treatment of social anxiety disorder. In some studies, Internet-delivered treatment can achieve similar outcomes as in face-to-face CBT, but the literature thus far is restricted mainly to efficacy trials. This article provides a brief summary of the evidence, comments on the role of the therapist and for which patient and therapist this is suitable. Areas of future research and exploration are identified.

    Objectives: Several studies have sought to analyze the cost-effectiveness of advanced home care andhome rehabilitation. However, the costs of informal care are rarely included in economic appraisals ofhome care. This study estimates the cost of informal care for patients treated in advanced home careand analyses some patient characteristics that influence informal care costs.Methods: During one week in October 1995, data were collected on all 451 patients in advanced homecare in the Swedish county of O¨ stergo¨ tland. Costs were calculated by using two models: one includingleisure time, and one excluding leisure time. Multiple regression analysis was used to analyze factorsassociated with costs of informal care.Results: Seventy percent of the patients in the study had informal care around the clock during theweek investigated. The patients had, on average, five formal care visits per week, each of which lastedfor almost half an hour. Thus, the cost of informal care constituted a considerable part of the costof advanced home care. When the cost of leisure time was included, the cost of informal care wasestimated at SEK 5,880 per week per patient, or twice as high as total formal caregiver costs. Whenleisure time was excluded, the cost of informal care was estimated at SEK 3,410 per week per patient,which is still 1.2 times higher than formal caregiver costs (estimated at SEK 2,810 per week per patient).Informal care costs were higher among patients who were men, who were younger, who had their ownhousing, and who were diagnosed with cancer.Conclusions: Studies of advanced home care that exclude the cost of informal care substantiallyunderestimate the costs to society, regardless of whether or not the leisure time of the caregiver isincluded in the calculations.

    Abstract [en]

    Background: Electronic-based alcohol screening and brief interventions for university students with problem drinking behaviours forms an important means by which to identify risky drinkers. Methods: In this study an e-SBI project was implemented to assess drinking patterns, and to provide personalised feedback about alcohol consumption and related health problems, to students in a Swedish university. In this study, third semester university students (n = 2858) from all faculties (colleges) at the University were invited to participate in e-SBI screenings. This study employed a randomised controlled trial, with respondents having a equal chance of being assigned to a limited, or full-feedback response. Results: The study shows that high risk drinkers tend to underestimate their own consumption compared to others, and that these high risk drinkers experience more negative consequences after alcohol intake, than other respondents. There was a strong belief, for both high-and low-risk drinkers, that alcohol helped celebrations be more festive. This study also confirms findings from other study locations that while males drank more than females in our study population; females reached the same peak alcohol blood concentrations as males. Conclusion: Obtaining clear and current information on drinking patterns demonstrated by university students can help public health officials, university administration, and local health care providers develop appropriate prevention and treatment strategies.

    This dissertation deals with how esthetic surgery as a phenomenon and as a pursuit has been constructed to attain cultural legitimacy, that is, how esthetic surgery has been presented, advocated, and shaped so as to make it seem a possible, reasonable, acceptable, and meaningful practice in the contemporary culture and in people-s lives. Temporally, the study period ranges from the early 1980s to the early 2000s. During that period, esthetic surgery made its breakthrough in Sweden as a practical pursuit and as a cultural phenomenon. Cultural legitimacy is a pivotal concept in the study. It is used here to denote that a pursuit or a phenomenon - in this case esthetic surgery - has attained a status in the public discourse that makes it seem conceivable and acceptable. In this dissertation, I study how this cultural legitimacy is established through expressions in media. I analyze the work of construction that is leading to esthetic surgery reaching that position from a focus on the rhetoric of legitimization, that is, how esthetic surgery is talked about so that cultural legitimacy can be attained. It is a matter of appealing to the public so that esthetic surgery is accepted as a body modification practice, available and relevant to everyone. The work with the material showed that creating justification, appeal, and credibility plays a prominent role in this construction. The promise inherent in esthetic surgery has to be presented as realistic and realizable, while simultaneously referring to culturally anchored values and defining these as associated with esthetic surgery. In this process justification, appeal and credibility have been linked to such dominating values as freedom, health and naturalness.

    Abstract [en]

    Background and Problem: It was shown that the lack of internal control was a common denominator among the reasons for the recent financial crisis. It was also one of the reasons that a Swedish bank in August 2010 went bankrupt and this can be seen as highly unexpected due to the developed rules, frameworks and reviews that exist for the Swedish banks today. In order for a company to accomplish a well functional internal control it must be integrated throughout the whole organization. In the current situation there is evidence confirming an existence of a gap between the internal control a board request and what is accessible to employees in the bank.

    Aim: The purpose of this thesis is to study how the four major Swedish banks are operating the internal control today, also, analyzing the communication and integration of the internal control within the organization. We also wish to contribute to an understanding of how internal control is integrated and how it becomes a part of the daily work in the bank.

    Method/Empirics: Since this study examines how communication and integration of internal control appears in the major Swedish banks, a purely qualitative approach with interviews of people who actively work with the internal control was not considered as sufficient. Therefore, the empirical data consists of interviews with employees who are actively working with internal control in the four major Swedish banks and additionally, a survey was sent to the employees working in the daily business.

    Conclusions: The work with internal control in the major Swedish banks is well functioning and it is possible to notice a tendency for increased focus for the subject. An overall assessment and comparison of responses from the interviews and the survey indicates a gap in communication regarding internal control. Thus, there is a lack of integration of the internal control.

    Ämnet för denna undersökning är de teologiska perspektiven runt två gudomligförklarade kungar i det forntida Mesopotamien, nämligen den gammalakkadiske kungen Naram-Sin (ca. 2254-2218 f.kr.) och den nysumeriske kungen Shulgi (ca. 2094-2047 f.kr.). De teologiska perspektiven behandlas här med hjälp av en induktionsbaserad historisk undersökning som utgår från vad olika forskare har skrivit i ämnet. Eftersom detta är en c-uppsats och det inte har varit möjligt att använda sig av primärkällor såsom arkeologiskt material och historiska källor på originalspråk så utgår denna undersökning uteslutande från forskningslitteratur. Undersökningen består av tre delar, i de två första delarna behandlas de teologiska perspektiven på kungarna var för sig och i det tredje görs en jämförelse mellan perspektiven. Undersökningens upplägg ser ut på så sätt att först har det gjorts observationer av vad som står om de teologiska perspektiven på monarkerna i de olika forskarnas verk. Sedan har perspektiven brutits ner i olika delämnen och därefter har informationen om dem sorterats in i de olika delämnena. Utifrån detta försöker sedan undersökningen att skissa fram en bild av de teologiska perspektivens karaktär. Uppsatsens tyngdpunkt ligger framförallt på att peka på olika tendenser i de religiösa föreställningarna runt dem med avseende på vilka teman de kretsar kring. I undersökningen framkommer det att perspektiven är ganska spretiga och går i många olika riktningar och även att det finns skillnader och likheter i perspektiven mellan kungarna. Två teman som dock ofta återkommer är fruktbarhet och himlen.

    Background: In ten to twelve years, 40 % of Sweden's 500 000 family- and owner led firms will be up for sale according to an estimation done by PwC in 2012. Many small and medium sized firms will therefore need to be valued within the near future. The models and theories used in todays' business valuations are not applicable for small private firms why the authors chose to conduct this study.

    Purpose: The purpose of this study is to design a practically useful model in terms of assessing the owners' required rate of return in a small private firm. The model should include the firm-specific risks that are linked to private firms.

    Methodology: This study uses a qualitative research method where data is collected through interviews with people who on a daily basis, work with, value and advise small private firms. From the data collected through the interviews as well as theories regarding the subject the authors have designed a practical model with the intentions to support private firm valuations.

    Conclusion: The BRACE-model (Business Risk Adjusted Cost of Equity) consists of two main components, a CAPM-premium and a firm-specific risk premium. The firm-specific risk premium is derived from the firm's size and illiquidity and is further quantified with a point system where a small private firm's specific risks are considered.

    Deciding Fast and Slow: How Intuitive and Reflective Thinking Influence Decision Making2016Doctoral thesis, comprehensive summary (Other academic)
    Abstract [en]

    Paper I “Intuition and cooperation reconsidered”: Does intuition make people more cooperative? Rand et al. (Rand, Greene, & Nowak, 2012) reported increased cooperation in social dilemmas after forcing individuals to decide quickly. We test the robustness of this finding in a series of five experiments involving about 2,500 subjects in three countries. None of the experiments confirms the Rand et al. (2012) finding, indicating that their result was an artefact of excluding about 50% of the subjects who failed to respond on time.

    Paper II “Intuition and moral decision-making – the effect of time pressure and cognitive load on moral judgment and altruistic behavior”: Do individuals intuitively favor certain moral actions over others? This study explores the role of intuitive thinking — induced by time pressure and cognitive load — in moral judgment and behavior. Overall we find converging evidence that intuitive states do not influence moral decisions. Across all samples and decision tasks men were more likely to make utilitarian moral judgments and act selfishly compared to women, providing further evidence that there are robust gender differences in moral decision-making.

    Paper III “Public views on policies involving nudges”: When should nudging be deemed as permissible and when should it be deemed as intrusive to individuals’ freedom of choice? The main objective of this study is to elicit public views on the use of nudges in policy. In particular we investigate attitudes toward two broad categories of nudges that we label pro-self (i.e. focusing on private welfare) and pro-social (i.e. focusing on social welfare) nudges. Results show that the level of acceptance toward nudge-policies was generally high. Nudge polices classified as pro-social had a significantly lower acceptance rate compared to pro-self nudges.

    Paper IV “The effect of fast and slow decisions on financial risk-taking”: Are individuals financial risk taking influenced by time available? We experimentally compare fast and slow decisions in a series of experiments on financial risk taking in three countries involving over 1,700 subjects. We find that time pressure increases risk aversion for gains and risk taking for losses compared to time delay; implying that time pressure increase the reflection effect of Prospect Theory.

    Paper V “Incidental effect and financial risk-taking – a neural investigation: This study builds on the results from Paper IV. Here I explore the influence of incidental negative emotions on financial risk-taking in an fMRI environment in order to assess underlying neural mechanisms. I experimentally compare neutral and unpleasant valence framing on gambles involving pure monetary gain and pure monetary loss. I find a significantly increased BOLD response in left amygdala and bilateral visual cortex when contrasting when showing unpleasant pictures, a neural effect which is in line with previous neuroimaging studies on negative emotions. However the neural effect of showing unpleasant pictures did not affect choices in the risk tasks. Consequently, I did not find any support for the hypothesis that the reflection effect of Prospect theory should be more pronounced when making risky choices influenced by incidental negative emotions.

    Economic effects and social implications of a dual currency and exchange rate system: A study of the Cuban case2011Independent thesis Advanced level (degree of Master (Two Years)), 20 credits / 30 HE creditsStudent thesis

    In 1994 the Cuban regime started a gradual introduction of a new currency, the convertible peso (CUC), which could be exchanged for USD at par. In the beginning the circulation of CUC was limited, but in 2003 the CUC replaced the USD in state owned enterprises and in 2004 the USD was replaced by the CUC in all transactions made by the population and foreigners in Cuba. Two exchange rates also exist between these currencies, one for the population (unofficial exchange rate of 24:1) and one for companies and institutions (official exchange rate of 1:1). Both currencies suffer from a lack of convertibility and have created an unusual and segmented monetary environment in Cuba.

    The aim of the study is to analyze the effects of the dual currency and exchange rate in the economy in terms of economic costs and in terms of how the capabilities of the population are affected. In order to do this, a Minor Field Study was conducted in Havana where different agents of the Cuban economy were interviewed and available data collected.

    The results show that the dual monetary system creates several distortions in the Cuban economy. The dual exchange rate implicitly subsidizes imports and at the same time creates an indirect tax on exports. A system of converting both domestic currencies to one “unique currency” further complicates calculation of the productivity and profitability of state owned companies. Additionally, capital controls and segmentations of markets weaken the connections between foreign and Cuban companies and also decrease much needed inflow of foreign currency. Capital controls also leads to Cuban companies being restricted diachronically in the use of their capital in that way hampering necessary investment.

    Furthermore, the dual monetary system affects the Cuban people’s capabilities, which to a large extent depend on the direct access to CUC. Direct access to CUC in turn does not depend on productivity but on factors outside of a person’s control, such as having family abroad or being self-employed. Cuban’s with only a salary in CUP and without access to CUC has difficulties reaching a reasonable standard of living. This creates the perception that someone with direct access to CUC is richer than someone without, which leads to rent-seeking activities with people trying to work in the CUC sector even if they are highly educated and qualified for important occupations in the CUP sector.

    Abstract [en]

    Background

    Depressive disorders have been associated with a number of co-morbidities, and we   hypothesized that patients with a depression diagnosis would be heavy users of health   care services, not only when first evaluated for depression, but also for preceding   years. The aim of this study was to investigate whether increased health care utilisation   and co-morbidity could be seen during five years prior to an initial diagnosis of   depression.

    Methods

    We used a longitudinal register-based study design. The setting comprised the general   population in the county of Östergötland, south-east Sweden. All 2470 patients who   were 20 years or older in 2006 and who received a new diagnosis of depression (F32   according to ICD-10) in 2006, were selected and followed back to the year 2001, five   years before their depression diagnosis. A control group was randomly selected among   those who were aged 20 years or over in 2006 and who had received no depression diagnosis   during the period 2001-2006.

    Results

    Predictors of a depression diagnosis were a high number of physician visits, female   gender, age below 60, age above 80 and a low socioeconomic status.

    Patients who received a diagnosis of depression used twice the amount of health care   (e.g. physician visits and hospital days) during the five year period prior to diagnosis   compared to the control group. A particularly strong increase in health care utilisation   was seen the last year before diagnosis. These findings were supported with a high   level of co-morbidity as for example musculoskeletal disorders during the whole five-year   period for patients with a depression diagnosis.

    Conclusions

    Predictors of a depression diagnosis were a high number of physician visits, female   gender, age below 60, age above 80 and a low socioeconomic status. To find early signs   of depression in the clinical setting and to use a preventive strategy to handle these   patients is important.

    Human factors research popularly employs perception-based techniques to investigate team performance and its dependency to cognitive processes. Such studies frequently rely upon either observer-based or self-assessment techniques to collect data. In this study, we examined behavioral observer ratings and self-assessment ratings for measuring team performance in virtual teams, with team performance regarded as a combination of task outcome and team cognition. Juxtaposing self-assessments and observer ratings from a quasi-experiment comparing team performance rating techniques reveals that they indeed produce overall similar results, with both singling out teamwork effectiveness ratings as the strongest contributor to overall team performance. However, the comparisons show remarkably low correlation on individual questionnaire items. The most striking difference is that the team members self-assessments of workload are lower than the corresponding observer ratings. In particular, the self-assessments do not correlate at all with overall team performance, whereas the observers workload ratings are more consistent with contemporary research that suggests a strong correlation between workload and team performance, suggesting that observer-based techniques are more reliable than self-assessments for assessing workload. For other ratings, the results show that the two techniques are fairly equal, suggesting that the choice between methods to employ can be deferred to other considerations such as obtrusiveness, accessibility, and resource availability.

    Background. Cognitive behaviour therapy (CBT) is an effective treatment for obsessive-compulsive disorder (OCD) but access to CBT is limited. Internet-based CBT (ICBT) with therapist support is potentially a more accessible treatment. There are no randomized controlled trials testing ICBT for OCD. The aim of this study was to investigate the efficacy of ICBT for OCD in a randomized controlled trial. less thanbrgreater than less thanbrgreater thanMethod. Participants (n=101) diagnosed with OCD were randomized to either 10 weeks of ICBT or to an attention control condition, consisting of online supportive therapy. The primary outcome measure was the Yale-Brown Obsessive Compulsive Scale (YBOCS) administered by blinded assessors. less thanbrgreater than less thanbrgreater thanResults. Both treatments lead to significant improvements in OCD symptoms, but ICBT resulted in larger improvements than the control condition on the YBOCS, with a significant between-group effect size (Cohens d) of 1.12 (95% CI 0.69-1.53) at post-treatment. The proportion of participants showing clinically significant improvement was 60% (95% CI 46-72) in the ICBT group compared to 6% (95% CI 1-17) in the control condition. ICBT is an efficacious treatment for OCD that could substantially increase access to CBT for OCD patients. Replication studies are warranted.

    BACKGROUND: As relapse after completed cognitive behavior therapy (CBT) for obsessive-compulsive disorder (OCD) is common, many treatment protocols include booster programs to improve the long-term effects. However, the effects of booster programs are not well studied. In this study, we investigated the long-term efficacy of Internet-based CBT (ICBT) with therapist support for OCD with or without an Internet-based booster program.

    METHOD: A total of 101 participants were included in the long-term follow-up analysis of ICBT. Of these, 93 were randomized to a booster program or no booster program. Outcome assessments were collected at 4, 7, 12 and 24 months after receiving ICBT.

    RESULTS: The entire sample had sustained long-term effects from pre-treatment to all follow-up assessments, with large within-group effect sizes (Cohen's d = 1.58-2.09). The booster group had a significant mean reduction in OCD symptoms compared to the control condition from booster baseline (4 months) to 7 months, but not at 12 or 24 months. Participants in the booster group improved significantly in terms of general functioning at 7, 12 and 24 months, and had fewer relapses. Kaplan-Meier analysis also indicated a significantly slower relapse rate in the booster group.

    CONCLUSIONS: The results suggest that ICBT has sustained long-term effects and that adding an Internet-based booster program can further improve long-term outcome and prevent relapse for some OCD patients.

    Uppsatsens syfte är att undersöka hur skapande människor beskriver inspiration i sina skapandeprocesser genom att analysera texter skrivna utav fem olika skapande människor själva. Birger Gerhardsson (2014) har skrivit i Nationalencyklopedin att inspiration är ett andligt fenomen som övergår människans förmåga. Vidare i uppsatsen beskrivs den tidigare forskning och teori som funnits inom inspiration och kreativitet. Metoden som använts är textanalys och det för att kunna analysera vad det är författarna verkligen menar. Uppsatsen tar även upp kreativitet i relation till inspiration då de är två begrepp som relaterar till varandra. I resultatanalysen har Helene Billgren (2011) skrivit om hur hon kan titta på klippdockor för att hitta inspiration medan flera av de andra författarna inte har skrivit om inspiration med just det begreppet utan har skrivit mer om sina skapandeprocesser på ett sätt som går att tolka som inspiration. I summeringen av resultatanalysen beskrivs kortfattat vad uppsatsens resultat är, bland annat att det finns många olika sätt att beskriva inspiration och att alla beskrivningar kommer ifrån olika utgångspunkter. Diskussionsdelen diskuterar och argumenterar kring varför valet av metod var bra, hur frågeställningen blev besvarad och hur en vidare forskning på ämnet inspiration skulle kunna se ut.

To fulfill the purpose of the study, fifteen qualitative interviews were conducted with recruiters on Spotify's recruitment department Talent Acquisition.

    The result shows a positive experience of flexibility and accessibility, but also shows an awareness of the challenges of constant connectivity, flexibility and accessibility and how they affect employees' work-life balance, and that boundaries tend to become blurred and more diffuse. This awareness has resulted in several individual strategies to handle the challenges of boundaryless work.

    Abstract [sv]

    Syftet med detta examensarbete är att synliggöra vilka uppfattningar lärare och elever har om lust och motivation i textilslöjden. Forskning visar att slöjd upplevs som ett lustfyllt ämne och att lustfyllda situationer påverkar elevers inlärning och utveckling positivt. Frågeställningarna i detta examensarbete syftar därför till att synliggöra lärares och elevers uppfattningar om lustens roll i textilslöjdundervisning, orsaker till lust och motivation samt strategier för att skapa lust och motivation i textilslöjden.

    För att svara på dessa frågor har en undersökning med kvalitativ ansats utförts. Intervjuer med fyra lärare och fem elever på skolor i grundskolans senare år och på gymnasiet har analyserats och resulterat i flera kategorier som visar på lärares och elevers uppfattningar.

    Dessa kategorier beskriver uppfattningar kring lustens och motivationens roll i textilslöjdundervisning. De intervjuade menar att lust gör det lättare att lära sig. Variationen av praktisk-estetiska och teoretiska ämnen skapar lust och motivation. I lustfylld textilslöjdundervisning driver elever arbetet framåt själva, de har stort medbestämmande, arbetet flyter på och de klarar av utmaningar. Orsaker till lust och motivation är inspiration, inflytande, intresse, problemlöning, görandet, visionen av en produkt, att få med sig något hem, att bli nöjd med resultat, att ha kontakt med andra, och en kunnig lärare. När lust och motivation saknas används strategierna att ta en paus, att sätta igång fast man inte har lust, hitta lust och motivation själv, att söka eller ge hjälp samt att vara positiv.

    Background: In recent years many business environments have been characterized by a fast and advanced technical development, change in demands and increased competition. The market conditions are constantly changing and because of that organizations have to find new ways to compete. This can be realized through adjustments and restructurings of the firm’s resources portfolio and it is also called dynamic capabilities. Key Words in this process are timing, speed and flexibility. Nevertheless, these elements can be hard to attain since many organizations have an internal inertia that will restrict their agility.

    Aim: The purpose of the study is to analyze how an organization can develop dynamic capabilities and in the same time reduce its internal inertia.

    Method: To answer the above reasoning a case study has been realized at a Swedish company within the sector of horse-breeding. The stud farm Flyinge AB was chosen and five different interviews were conducted. An additional interview was conducted at Hästak AB with the purpose to get a better understanding of the horse-breeding industry.

    Result: Change can be seen as a complex process with many different factors that affect the outcome. A dynamic capability is a process of change and it can be either episodic or continuous. The ideal form of change is continuous since it has a low level of internal inertia. The resistance to change can be seen in organizational structure and culture. To eliminate these obstacles the leader has an important role. The leader should identify the problem, come up with a strategy, communicate the new strategy/change, find acceptance for it and finally implement it. In the change process balanced scorecard can be used as an instrument for communications.

    IMPORTANCE: It is unclear whether d-cycloserine (DCS), a partial N-methyl-d-aspartate agonist that enhances fear extinction, can augment the effects of exposure-based cognitive behavioral therapy (CBT) for obsessive-compulsive disorder (OCD).

    OBJECTIVES: To examine whether DCS augments the effects of CBT for OCD and to explore (post hoc) whether concomitant antidepressant medication moderates the effects of DCS.

    DESIGN, SETTING, AND PARTICIPANTS: A 12-week, double-blind randomized clinical trial with 3-month follow-up conducted at an academic medical center between September 4, 2012, and September 26, 2013. Participants included 128 adult outpatients with a primary diagnosis of OCD and a Yale-Brown Obsessive Compulsive Scale (Y-BOCS) score of 16 or higher. Concurrent antidepressant medication was permitted if the dose had been stable for at least 2 months prior to enrollment and remained unchanged during the trial. The main analysis was by intention-to-treat population.

    INTERVENTIONS: All participants received a previously validated Internet-based CBT protocol over 12 weeks and were randomized to receive either 50 mg of DCS or placebo, administered 1 hour before each of 5 exposure and response prevention tasks.

    MAIN OUTCOMES AND MEASURES: Clinician-administered Y-BOCS score at week 12 and at 3-month follow-up. Remission was defined as a score of 12 or lower on the Y-BOCS.

    RESULTS: In the primary intention-to-treat analyses, DCS did not augment the effects of CBT compared with placebo (mean [SD] clinician-rated Y-BOCS score, DCS: 13.86 [6.50] at week 12 and 12.35 [7.75] at 3-month follow-up; placebo: 11.77 [5.95] at week 12 and 12.37 [6.68] at 3-month follow-up) but showed a significant interaction with antidepressants (clinician-rated Y-BOCS, B = -1.08; Z = -2.79; P = .005). Post hoc analyses revealed that antidepressants significantly impaired treatment response in the DCS group but not the placebo group, at both posttreatment and follow-up (clinician-rated Y-BOCS: t62 = -3.00; P = .004; and t61 = -3.49; P < .001, respectively). In the DCS group, a significantly greater proportion of antidepressant-free patients achieved remission status at follow-up (60% [95% CI, 45%-74%]) than antidepressant-medicated patients (24% [95% CI, 9%-48%]) (P = .008). Antidepressants had no effect in the placebo group (50% [95% CI, 36%-64%] remission rate in both groups).

    CONCLUSIONS AND RELEVANCE: The findings suggest that antidepressants may interact with DCS to block its facilitating effect on fear extinction. Use of DCS may be a promising CBT augmentation strategy but only in antidepressant-free patients with OCD.

    Abstract [en]

    Obsessive-compulsive disorder (OCD) is a common and disabling disorder. Although evidence-based psychological treatments exists, such as cognitive behavior therapy (CBT), the cost-effectiveness of CBT has not been properly investigated. In this trial, we used health economic data from a recently conducted randomized controlled trial, where 101 OCD patients were allocated to either internet-based CBT (ICBT) or control condition (online support therapy). We analyzed treatment effectiveness in relation to costs, using both a societal- (including all direct and indirect costs) and a health care unit perspective (including only the direct treatment costs). Bootstrapped net benefit regression analyses were also conducted, comparing the difference in costs and effects between ICBT and control condition, with different willingness-to-pay scenarios. Results showed that ICBT produced one additional remission for an average societal cost of $931 and this figure was even lower ($672) when narrowing the perspective to treatment costs only. The cost-utility analysis also showed that ICBT generated one additional QALY to an average price of $7186 from a societal perspective and $4800 when just analyzing the treatment costs. We conclude that ICBT is a cost-effective treatment and the next step in this line of research is to compare the cost-effectiveness of ICBT with face-to-face CBT. (C) 2014 Elsevier Inc.     Internet-based cognitive behavior therapy (ICBT) for obsessive-compulsive disorder (OCD) has shown efficacy in randomized trials but many patients do not respond to the treatment, we therefore need to find predictors and moderators of treatment response. In this study, we analyzed predictors of ICBT response using both post-treatment as well as 24-month outcome data. As half of the participants were randomized to receive an Internet-based booster program as an adjunct to ICBT, we also investigated moderators of ICBT with or without booster. Results showed that more severe baseline OCD symptoms predicted worse end state outcome but also higher degree of change. Furthermore, high degree of working alliance predicted better outcome but patients with primary disgust emotions had worse treatment effects. The moderator analysis also indicated that scoring high on the obsessing subscale on the Obsessive-Compulsive Inventory-Revised predicted worse treatment outcome in the booster group. In conclusion, there are some possible predictors and moderators of ICBT for OCD but more research is needed with larger and clinically representative samples. (C) 2014 Elsevier Inc.     Although cognitive interventions for obsessive-compulsive disorder (OCD) have been tested in randomized trials, there are few trials that have tested the specific mechanisms of cognitive interventions, i.e. how they achieve their effects. In this study, we aimed to investigate the mediating effects of a short cognitive intervention in the treatment of OCD and used data from a recently conducted randomized controlled trial where 101 participants were allocated to either Internet-based CBT (ICBT) or to a control condition. Obsessive beliefs were measured at pre-treatment, at the time they had received the cognitive intervention, and also at post-treatment. Weekly OCD symptoms were measured throughout the 10 weeks of treatment. We hypothesized that (1) the ICBT group would have greater reductions in obsessive beliefs (controlling for change in OCD symptoms) after completing the cognitive intervention, and that (2) this reduction would, in turn, predict greater OCD symptom reduction throughout the rest of the treatment period. Contrary to our expectations, the longitudinal mediation analysis indicated that (1) being randomized to ICBT actually increased the degree of obsessive beliefs after receiving the cognitive intervention at weeks 1-3, and (2) increase in obsessive beliefs predicted better outcome later in treatment. However, when repeating the analysis using cross-sectional data at post-treatment, the results were in line with the initial hypotheses. Results were replicated when the control condition received ICBT. We conclude that, although obsessive beliefs were significantly reduced at post-treatment for the ICBT group, early increase rather than decrease in obsessive beliefs predicted favourable outcome.     Background: Cognitive behavior therapy (CBT) is widely regarded as an effective treatment for obsessive compulsive disorder (OCD), but access to CBT therapists is limited. Internet-based CBT (ICBT) with therapist support is a way to increase access to CBT but has not been developed or tested for OCD. The aim of this study was to evaluate ICBT for OCD. less thanbrgreater than less thanbrgreater thanMethod: An open trial where patients (N = 23) received a 15-week ICBT program with therapist support consisting of psychoeducation, cognitive restructuring and exposure with response prevention. The primary outcome was the Yale-Brown Obsessive Compulsive Scale (Y-BOCS), which was assessed by a psychiatrist before and immediately after treatment. Secondary outcomes were self-rated measures of OCD symptoms, depressive symptoms, general functioning, anxiety and quality of life. All assessments were made at baseline and post-treatment. less thanbrgreater than less thanbrgreater thanResults: All participants completed the primary outcome measure at all assessment points. There were reductions in OCD symptoms with a large within-group effect size (Cohens d = 1.56). At post-treatment, 61% of participants had a clinically significant improvement and 43% no longer fulfilled the diagnostic criteria of OCD. The treatment also resulted in statistically significant improvements in self-rated OCD symptoms, general functioning and depression. less thanbrgreater than less thanbrgreater thanConclusions: ICBT with therapist support reduces OCD symptoms, depressive symptoms and improves general functioning. Randomized trials are needed to confirm the effectiveness of this new treatment format.

    Cognitive behavior therapy (CBT) is an effective treatment for OCD when delivered face-to-face, in group-format and also via the internet. However, despite overall large effect sizes, a considerable amount of the patients relapse. One intervention that has the potential to reduce these relapse rates is booster programs, but if booster program is a cost-effective method of preventing relapse is still unknown. We used health economical data from a recent randomized controlled trial, where patients who had undergone an internet-based CBT were randomly allocated to receive an additional booster program. Assessment points were 4-, 7-, 12- and 24-month. Health economical data were primarily analyzed using a societal perspective. Results showed that the booster program was effective in preventing relapse, and the cost of one avoided relapse was estimated to $1066-1489. Cost-effectiveness acceptability curves showed that the booster program had a 90% probability of being cost-effective given a willingness to pay of $1000-1050 the first year, but this figure grew considerably after two years ($2500-5500). We conclude that internet-based booster programs are probably a cost-effective alternative within one-year time frame and that more treatment may be needed to maintain adequate cost-effectiveness up to two years.     Abstract [en]

    Background: Irritable Bowel Syndrome (IBS) is highly prevalent and is associated with a substantial economic burden. Cognitive behavior therapy (CBT) has been shown to be effective in treating IBS. The aim of this study was to evaluate the cost-effectiveness of a new treatment alternative, internet-delivered CBT based on exposure and mindfulness exercises. Methods: Participants (N = 85) with IBS were recruited through self-referral and were assessed via a telephone interview and self-report measures on the internet. Participants were randomized to internet-delivered CBT or to a discussion forum. Economic data was assessed at pre-, post- and at 3-month and 1 year follow-up. Results: Significant cost reductions were found for the treatment group at $16,806 per successfully treated case. The cost reductions were mainly driven by reduced work loss in the treatment group. Results were sustained at 3-month and 1 year follow-up. Conclusions: Internet-delivered CBT appears to generate health gains in IBS treatment and is associated with cost-savings from a societal perspective.
